City of Phoenix Mayor Kate Gallego and Phoenix Center for the Arts are excited to announce the Ninth Annual Mayor’s Arts Awards will return to Hance Park Urban Plaza on November 18, 2021.
An evening dedicated to recognizing Phoenix’s outstanding arts leaders, this fundraising event benefits Phoenix Center for the Arts in support of our mission to provide arts and culture opportunities to all.

Health & Safety
In the interest of health and safety, all Mayor’s Arts Awards guests will be required to provide proof of a negative COVID-19 diagnostic test (taken within 72 hours prior to the event), OR be fully vaccinated* against COVID-19 and provide proof of COVID-19 immunization.
*As defined by the CDC, people are considered fully vaccinated:

• 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or
• 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine

To enter the event, you will need a government-issued ID plus either:

• A vaccination card indicating full vaccination, or
• A negative test result dated within 72 hours prior to the event.

Masks will be required during check-in and strongly encouraged throughout the event when not eating or drinking.
Allowable proofs of vaccination and/or negative test status include:

• A physical vaccination card
• A picture of your vaccination card (either hard copy or stored on your phone)
• A digital confirmation of vaccination status through a vaccination passport app
• A copy of your official immunization record as issued by a state department of health services
• Proof of negative COVID-19 PCR (polymerase chain reaction) test result within 72 hours of the event. The test must be issued by a medical provider or pharmacy. At-home tests will not be accepted. Antigen and/or antibody test results will not be accepted.

Required documentation will be checked at the entrance door by security personnel. Entry will be denied for guests who do not provide documentation as required above.

En aras de la salud y la seguridad, todos los invitados a los Mayor’s Arts Awards deberán presentar una prueba de diagnóstico de COVID-19 negativa (tomada dentro de las 72 horas anteriores al evento), O estar completamente vacunados* contra el COVID-19 y presentar una prueba de inmunización contra el COVID-19.

La prueba debe ser emitida por un proveedor médico o una farmacia. No se aceptarán las pruebas caseras. No se aceptarán los resultados de las pruebas de antígenos y/o anticuerpos.

La documentación requerida será inspeccionada en la puerta de entrada por el personal de seguridad. Se denegará la entrada a los huéspedes que no presenten la documentación requerida anteriormente.
